discuss the fault in the GM ignition switch

I need to do a paper about this issue as a quality engineer. like what was the cause of the problem and if they could avoid it from the beginning and if it was managers fault or design fault and how many cars they did recall and how they fixed it and what was the cost of this small problem.

I just need a couple of papers with references.
