Maria

You are a psychologist working in a women’s unit for a substance abuse agency as a member of the treatment team. Your specific job is to conduct individual sessions with clients who have problems that are untreatable in group therapy. Typically, you provide mental health services for clients who do not have another psychologist, and your clients are very needy. In fact, virtually all of your clients are unemployed and are quite poor.

During Maria’s appointment, you learn that she has just discovered that she is 6 weeks pregnant, and despite her continued drinking and use of cigarettes, she thinks that her baby will be healthy. The agency nurse asked you to talk to her about how her behaviors are likely endangering the baby. She has explained the physical problems, but Maria responds better to you. Maryam is a 17-year-old Caucasian female university student who was referred to your agency by her physician Dr. Jaffee. Maryam presented in her doctor’s office complaining of lack of sleep. Dr. Jaffee did not give her medication as Maryam has reported drinking three to four glasses of vodka and orange juice per night to sleep. Dr. Jaffee’s report indicates the patient is sleeping 2-4 hours per night and often awakens with nightmares. Blood tests were normal with the exception of slightly elevated liver enzymes. Blood pressure was 130/94. Patient was scheduled for a follow-up appointment in two weeks.

Behavioral Observations

Maryam arrived on time for her appointment. She was driven to the appointment by her university roommate. The client appeared anxious, had circles under her eyes, and was tearful during the intake. Maryam was oriented to time, place, and person. Client vocabulary was above average. Client appeared tired and despondent evidenced by low voice, soft speech, and flat affect.

Presenting Problem

Maryam states “I can’t drive a car. I am too afraid that someone will hit me or I will hit someone else. I can’t sleep so I don’t get up to go to class in the morning.” Client states, “I am afraid to drive.” She goes on to state, “I made such a horrible mistake; I don’t deserve to live. I am so stupid.”

Client states that she was involved in a three car accident two months ago. Client reports one person was critically injured and the other was treated and released at the hospital. Client reports she received a citation, as she turned in front of two oncoming cars at a red light.

There is a lot of room for interpretation here, so there isn’t necessarily one right answer. Mitchell Dawson and three of his friends purchased nonrefundable tickets from Live Nation Entertainment to attend a concert at the Straz Center in Tampa.  The front of the ticket included a printed statement that the price included a $10 parking fee.  Dawson and his friends hired an Uber driver to take them to the concert.

Frustrated at being charged for parking that he did not need, Dawson filed a lawsuit in federal district court against Live Nation arguing that the bundled parking fee was unfair since consumers were forced to pay it in order to attend the concert.  He asserted the tying arrangement violated Section 1 of the Sherman Act.

A new type of third-party-reimbursement healthcare payment plan is emerging in the United States. CDHPs strive to control costs and improve quality of care by requiring consumers to take control of their own healthcare decisions. Consumers decide how they want to spend their healthcare dollars, depending on what is important to them. CDHPs are geared to encourage participants to enroll in some type of wellness program and improve their lifestyles. Specific types of CDHPs are health reimbursement accounts (HRA), flexible spending accounts (FSA), and health savings accounts (HSA).

However, there are concerns about CDHPs. The consumer may neither understand nor have the education and the tools to manage his or her own healthcare appropriately. This may have long-term ramifications on the whole healthcare system and whether CDHPs can be successful for the consumer, the employer, the physician, and the healthcare facilities, as well as the insurers. Answer the following questions in regard to this development:

•Summarize the history of when, how, and why CDHPs were developed.

•Explain HSA, HRA, and FSA with examples.

•Examine different segments of the population. Describe which socioeconomic group is likely to benefit the most from CDHPs.
