Case Study

Rosie is a 28-year-old single mother to Sam, her 6-year-old son. Sam was diagnosed with Neuroblastoma which is a form of cancer that is made up of cells that are found in nerve tissues of the body at the age of 4 and has been receiving a variety of known treatment options throughout this time inclusive of Chemotherapy which Sam is currently undergoing. Rosie has no current contact with any of her family members due to her traumatic childhood.

This has been a long journey for their family and Rosie has consistently struggled with watching Sam decline health wise throughout treatment and constantly analyses Sam’s quality of life. Sam cannot play sport of any kind, at times due to his level of unwellness he cannot hold a book at bedtime, he has not commenced school due to his treatment therefore has minimal friends to support him throughout this time period. Sam became extremely unwell during his last round of treatment which again provoked Rosie to assess how this medical intervention was helping her son.

Following Sam’s last round of treatment, scans took place which concluded the Neuroblastoma had again spread throughout Sam’s body however Doctors were confident that Sam would be eligible for a new drug trial. Following Rosie’s assessment of Sam who is undernourished and unwell from the impact of chemotherapy she has declined any further treatment for Sam and has requested support to prepare for Sam’s death with the inclusion of the Palliative care team.

Attention is choosing and processing a specific amount of information from all of the information that is being absorbed by our senses, stored memories and other cognitive processes (Sternberg & Sternberg, 2017, p. 119). The four main areas of attention are signal detection and vigilance, search, selective attention and divided attention. Although they are each very important depending on the circumstances, selective attentions stands out to be the most important for cognition. For cognitive processes to happen, a person has to be able to pay attention to and focus on something for a certain amount of time.

There are several “filter and bottleneck” theories to help explain selective attention. A filter blocks some of the information going through, selecting only a part of it while the bottleneck slows down the information that is passing through (Sternberg & Sternberg, 2017, p. 128). This can be explained more with the Early Filter Model by Donald Broadbent which says that all information comes through and is stored in sensory memory for a very short period of time. It is then moved forward to a filter that will only allow one message to move forward again and be processed in more detail. From our short-term memory, we can pick and choose which information we want to keep and use later.

This is important for cognitive processing because if we were not able to selectively focus on certain information while other conversations around us are happening, it would be too hard to process the information and store it for later use. A weakness of this is that sometimes the information that is picked up on is due to the loudness, pitch, accent (Sternberg & Sternberg, 2017) or sometimes whatever information sounds more exciting versus what is more important. This is known as the cocktail party problem. For those who are not very good with divided attention, selective attention is very important when driving past an accident. At the same time, divided attention is more important for drivers than selective because there is always a lot to look out for while driving.

Levels and applications of qualitative research evidence.

Evidence-based social work practice calls for the use of research data to guide the development of social work interventions on the micro, mezzo and/or macro-levels. Kearney (2001) described ways qualitative research findings can inform practice. Qualitative findings can help social workers understand the clients’ experiences and “what it may feel like” (Kearney, 2001). Therefore, social workers can develop clinical interventions that take into account the experiences of their clients. Qualitative findings can also help social workers monitor their clients. For example, if after reading a qualitative study on how domestic violence survivors respond to stress, they can monitor for specific stress behaviors and symptoms (Kearney, 2001). In addition, they can educate their client what stress behaviors to look for and teach them specific interventions to reduce stress (Kearney, 2001)

 

Given the increasing diversity that characterizes the landscape in the United States, social workers need to take into account culture when formulating interventions. Social workers can utilize qualitative findings to plan interventions in a culturally meaningful manner for the client.

When developing a budget, what variables do you have to take into account? In health care organizations, two of the largest groups of factors that you must consider are first, volume, and second, staffing and supply. The Vignette of Marcus

The Vignette of Marcus: Marcus is a 10-year-old boy who has been struggling at school. During the past year he has frequently been distracted in class, and fidgety while in his seat. He often gets into trouble for not paying attention to the teacher, and has difficulty engaging in solitary activities such as reading or taking tests. He gets into trouble in school for talking too much, and for talking out of turn. While at recess, he continues to have difficulties standing still and taking turns. During class time, Marcus has difficulty following instructions and completing his work. He is easily distracted, does not pay attention to details, and frequently leaves his seat to interrupt the work of other students.

Marcus continues to have difficulty at home. He spends the weekdays with his mother and two younger brothers whose ages are 5 and 7. He tries to help with many of the household chores his mother cannot tend to while she is at work full-time, but has difficulty following through. He seems to want to be helpful. He has some difficulty getting along with his brothers, and often will take their toys without asking. On the weekends, Marcus lives with his father, stepmother, his 11-year-old stepsister, and his 3-year-old half-sister. He describes his father and stepmother as being very strict, which causes some difficulty for Marcus. He has difficulties sitting still, which causes significant difficulty when he goes out to dinner or to other events with his father and stepmother.

Marcus’ teacher has met with his parents on several occasions to discuss his poor academic progress. She has expressed concern with the level of his academic skills in writing and math, as well as with his ability to control his energy and to ‘fit in’ with the others in his class. Both the teacher and school principal have recommended counseling for Marcus, and the parents’ insurance plan will pay for 10 visits.
